If you are connecting with me then it is my assumption: 

that, 

1. You are dead serious. 

2. You are determined to do the startup.

3. You want to move forward with least waste of time in the process.

4. You have won your fears, uncertainties but have some doubts churning your mind rigorously.

This will enable both of us to quantify mutually about our ethos, aspirations and synergies.

Just talking (before doing your due diligence) is like randomly procuring vegetables or groceries, the after effect of which we just don't bother; like what we have eaten the day before yesterday. 

But moving forward as cofounder; we have to align for at least a couple of decades or may be just few months.

To understand the significance of filling that form try to contemplate the following questions.

1. Why do we want to disrupt the process?

2. To convert our time into thousand fold wealth, should both of us not follow some concrete steps; to protect our combined intellectual property?

3. Why do we not want to put some safeguards; to protect the future potential & wellbeing of our child (the start-up)?
